Social Worker/Neighborhood Life Coordinator
The person will be responsible for developing neighborhoods and supervising staff through establishing and maintaining a sense of home by eliminating institutional barriers and enhancing relationships between residents, staff and visitors. The NLC/Social Worker will maintain direct social service responsibilities for skilled and memory support residents.
- Conduct assessment of resident psychosocial needs, development of plan to meet resident's needs, implementation of plan, documentation which meets all regulations, and participation in the interdisciplinary care plan meetings.
- Works with household team to promote quality care and satisfaction for residents, staff and families by supporting person centered care.
- Holds staff members accountable to complete tasks in the neighborhood.
- Oversees the collection and documentation of Social Service information in accordance with federal, state, accreditation, and facility standards.
- Involved with hiring, development, coaching, and supervision of the nursing assistant team members.
Nursing staff follows a Resident Centered Staffing Model with 12 hour shifts.
- Day Shift: 7:30AM - 8PM for NURSES
- Day Shift: 7:30AM - 7:30PM for CNAs
- Night Shift: 7:30PM - 8AM for NURSES
- Night Shift: 7:30PM - 7:30AM for CNAs
- In every pay period (14 days) there are seven 12-hour shifts instead of ten 8-hour shifts.
- The schedule is always the same and repeats itself every 14 days.
- No more working long stretches. Only one three-day stretch in a 14 day period.
- Every staff person is scheduled a long weekend (Friday-Saturday-Sunday) every fourteen days
DAY SHIFT & NIGHT SHIFT, IN HOUSE REGISTRY, C.N.A.:
Responsible to provide routine care on the transitional care unit and/or skilled/ memory support neighborhoods in accordance with established policies, procedures, and nursing standards, and as may be directed by the Unit or Shift Supervisor or the Director of Nursing Services, to ensure that the highest degree of quality resident care is maintained at all times.
DAY SHIFT RN Nursing Clinical Coordinator:
Will assume the authority and accountability of organizing and directing the unit’s activities on the Day Shift 7:30am – 8:00pm.. Will provide leadership to Nursing and C.N.A. staff to include coaching, balancing workload, disciplinary action, and overall direction. Will be responsible for monitoring the safety of the residents, conduct rounds, pass meds, admissions, and provide bed side care as needed. Will establish priorities, meet with families and interdisciplinary teams, and participate on committees as assigned. Will communicate unit’s activities to the DON keeping her informed of any situations that may arise on the Shift. Prior hospital and long time care experience preferred.
NIGHT SHIFT RN Nursing Clinical Coordinator:

You will act as a hands-on manager assisting team as needed and filling in on the unit as needed, along with participating in direct care nursing. Will provide leadership to Nursing and C.N.A. staff to include coaching, balancing workload, disciplinary action, and overall direction. Will be responsible for monitoring the safety of the residents, conduct rounds, pass meds, admissions, and provide bed side care as needed. Will establish priorities, meet with families and interdisciplinary teams, and participate on committees as assigned. Will communicate unit’s activities to the DON keeping her informed of any situations that may arise on the Shift. Prior hospital and long term care experience preferred.
DAY SHIFT LPN/Restorative Nurse for Assisted Living
Will work as a floor nurse during the day shift primarily Monday - Friday, possibility of some weekends. Oversee and implement the Restorative Program; responsible for documentation, equipment, supplies and vendor negotiations. Programs include oversight of walk to dine, bowel and bladder, ADL’s, identify other related programs to enhance current services offered to residents, provide leadership and direction to a staff of two Restorative Aides, provide training through education and in-service to staff and residents, work with interdisciplinary teams on resident care plans, respond to family inquiries in a timely manner.
Strongly preferred to have a current Certified Restorative Nurse with 1-2 years supervisory experience. Previous experience designing and implementing restorative programs a plus. Strong organizational and decision making skills. Ability to motivate and lead a team.
RESTORATIVE, C.N.A. FULL TIME
Responsible to provide restorative services on the transitional care unit in accordance with established policies, procedures, and nursing standards, and as may be directed by Shift Supervisor or the Director of Nursing Services, to ensure that the highest degree of quality resident care is maintained at all times. Must be restorative certified or trained. Schedule is Monday - Friday 6:30am - 2:30pm. May fill in as a C.N.A. in other areas as needed.

Crimson Pointe
LPN, In-House Registry:
Responsible for daily assessments of residents, med pass, treatments, and providing guidance and direction to staff. Will provide support and feedback to residents and family members as well as work closely with physicians, pharmacy and Director of Nursing. This position will provide vacation and sick coverage for staff on both the Day and PM shifts as needed.
C.N.A., IN-HOUSE REGISTRY:
Responsible to assist residents with ADLs and activities as needed. Opportunity to utilize time management, customer service, and multi-tasking skills.
